1. Please use the attached form if you are a Weber student and would like to nominate a student or staff member for a STAR.
2. The form is self-explanatory. There are suggested reasons for nominating a student or staff member on the form. Please review the suggested reasons. Also: Fill in all sections, including making sure your homebase teacher reviews and discusses your choice with you.
3. Your homebase teacher must pre-approve the form with a signature.
4. Once the form is signed, please submit to Mrs. McAuliffe either through your homebase folder (addressed to "Mrs. McAuliffe/Room 7" or by returning the form to Mrs. McAuliffe in room 7 before or after school.
