As per New York State Requirements:

  •  3 doses Polio
  •  3 doses DTP, DTaP
  •  3 doses Hepatitis B
  •  3 doses Haemophilus B (HIB) (under 15 months) or 1 dose after 15 months
  •  2 doses Varivax
  •  1 dose Tdap (11 years of age or older)
  •  2 Measles, 1 Mumps, 1 Rubella  or  2 MMR  or  1 MMR, 1 Measles
Also required: PPD (tuberculin test) within 1 year prior to the start of school for any child new to the country and/or district.
 

Please remember that your child will be excluded from school if New York State Laws are not met.
